All forms that must be on file before an athlete is allowed to participate in tryouts can be found on this page.  Please familiarize yourself, both parents and athletes, with the handbook and forms you will be expected to follow. 

The Sports Boosters and Athletic Department have set up physicals to be performed for all AIS students in the gym. This convenience costs $20, which is donated to the Sports Boosters for the continued development of AIS’s competitive attitude.  Physicals will take place as follows:

* Sports physicals are valid for one calendar year. You may choice to get your physical at any time over the summer with your regular doctor in order to avoid long lines at AIS the days the physicals are conducted.
