Athletic Forms and Documents
Important Note: Permission and Physical Form
All athletes in Middle School and Upper School must have a signed permission to participate and physical evaluation form on file in the athletic office before tryouts. Please download the permission and physical forms below and have them filled out before attending tryouts.
Athletic physicals will be given at AIS on Monday, August 13 and Tuesday, August 14. Physical evaluation times are from 8 a.m. to noon and 1 p.m. to 5 p.m. Please bring these forms to your physical evaluation and follow the schedule below. The cost is $20. All proceeds will go to he Boosters Club.

You can also have the physical evaluation form completed by your child's physician. Both forms can be downloaded below.
All forms must be on file before an athlete is allowed to participate in tryouts.
* Sports physicals are valid for one calendar year. You may choice to get your physical at any time over the summer with your regular doctor in order to avoid long lines at AIS the days the physicals are conducted.
